Background and purpose The Stroke S1F Battery (S1F) is a standardized neurobehavioral battery to assess the competency of patients at discharge. Several issues related to feasibility and generalizability have been raised regarding the use of this instrument in research and clinical practice. We analyzed its reliability using a clinical sample. Patients and methods Adult stroke survivors (N = 162) with a relatively short time after stroke (median = 18 days) were recruited from two hospitals and were assessed with the S1F in the admission phase. Acceptability, internal consistency, criterion-related validity, and the ability of the S1F to detect cognitive deficits were analyzed. Results The mean age was 68 years, 34% were women, and the majority were left-handed. The S1F was generally well accepted. Its internal consistency was good for five of the eight subtests and moderate for four subtests. Test-retest reliability was very high. Correlations with functional and cognitive measures were mostly moderate to low. The sensitivity to detect patients with cognitive deficits was good, but the specificity was relatively low, reflecting the prevalence of cognitive impairment in the population of stroke patients. Conclusions This study provides validation for the use of the S1F in the assessment of patients at discharge in stroke rehabilitation. This instrument can be used in Brazilian hospitals or in multicentered studies.
